What Medical Records Are Needed To Be Approved For Medical Marijuana In Arkansas?

To qualify for medical marijuana treatment in Arkansas, a copy of any medical records which document that you are currently experiencing one of the 18 qualifying conditions will suffice. These documents can include hospital papers, clinic notes or any further evidence showing the condition. The more supporting documentation you can provide at your appointment, the better.

How Do I Get a Copy of My Medical Records In Arkansas?

In order to obtain a copy of your medical records, it is best to first reach out to your primary care physician. Simply call your primary physician or the office of the doctor in which you received your diagnosis and state that you are requesting a copy of your records. Most offices will likely require you to fill out a form upon your request. You should be able to pick up the form at the office or they can email, fax or mail it to you.

If your doctor's office does not have form, you can also write a letter requesting your medical records. Just be sure to include the following information:

  • Your full name

  • Date of birth

  • Social security number

  • Home address

  • Phone number

  • Email address

  • Record(s) being requested - stating your qualifying condition

  • The dates in which you were received the doctor’s care

  • You preferred method for delivery of your medical records (fax, email, pick-up, mail, etc.)
